چکیده

The technique of recording His bundle electrograms in man by a tripolar electrode catheter was applied in a study of patients with first, second, and third-degree heart block. In all patients with first-degree heart block (congenital, acquired, or induced by atrial pacing), the delay in A-V conduction occurred proximal to the His bundle as evidenced by a prolonged P-H interval. The enhancement of A-V conduction (shortening the P-H interval) due to isoproterenol was qualitatively similar in the three forms of first-degree block. In cases of Wenckebach phenomenon the P-H interval progressively increased until a dropped beat occurred. The nonconducted P wave was not followed by a His deflection, indicating block proximal to the His bundle. Cases of 2:1 and 3:1 block occurring proximal to the His bundle were also studied. The usefulness of Hisbundle recordings in the diagnosis of cases of complete and incomplete bilateral bundle-branch block is also demonstrated.

Retrograde conduction in complete atrioventricular block Study using His bundle recordings.

Using His bundle recordings and ventricular stimulation retrograde conduction was studied in 57 cases of complete AV block. VA conduction was observed in 13 patients. Of the 13, 12 had AV block distal to H, and during cardiac pacing 5 of them showed a retrograde His bundle potential. In all cases the AV node-His pathway was implicated in the transmission of the retrograde impulse. There was no ...

A study of atrioventricular conduction in atrial fibrillation and flutter in man using His bundle recordings.

Seven patients with atrial fibrillation and six patients with atrial flutter were studied using the technic of His bundle recordings. All 13 patients received digitalis. In atrial fibrillation each QRS complex (except for premature ventricular beats) was preceded by a single His bundle deflection. Complete block distal to the His bundle was not observed. Thus, the zone of concealment could be l...

Bundle branch and ventricular activation in man: a study using catheter recordings of left and right bundle branch potentials.

His (H), right bundle branch (RB), and left bundle branch (LB) potentials were recorded by electrode catheters passed into the left and right ventricles in seven patients without conduction defects. In another three patients, H and LB were recorded. Initial ventricular activation was considered to coincide with the onset of the QRS (Q). The following intervals were measured in milliseconds: H-Q...

Studies on ventriculo-atrial conduction and the reentry phenomenon.

SUMMARY Ventriculo-atrial conduction and the reentry phenomenon was studied during right ventricular and bundle of His pacing in the intact dog heart. Bipolar electrogram recordings were obtained from the sinus node, Bachman's bundle, the right and left atria, the bundle of His, and the ventricle. Retrograde conduction resulted in a specific change in the sequence of atrial activation. The reco...
